Island Airways in Michigan operates four Islanders. I don't know if they are the largest operator of Islanders in the US but they are up there.

There are 24 Britten Norman Islanders registered in the US and all are listed as separate owners.

Idk, it's a hard question because many are private/part 121 or part 135.

But worldwide, It would probably be British Airways and its regional affiliates.
