Axis I diagnoses include the following: Disorders evident form infancy/childhood/adolescence. (ADHD< eating disorders, tourette's) , Organic Mental Disorders ( Dementia arising from Alzheomer's,basically anyhting that has a physical medical cause ie a brain tumor), Psychoactive Substance Use Disorders ( drug dependence), Psychosis (schizophrenia,) Mood Disoders ( depression, bi-polars disorder) Anxiety/ Neuroses ( panic disorder, phobias, OCD, PTSD,) Somatorform Disoders, Dissociative Disorders (Mulitple Personality Disorder, psychogenic fugue), Sexual Disorders (exhibitionism, fetishism, etc) Sleep Disorders
Axis II refers to the following : Personality Disorders ( paranoid, avoidant, obsessice compulsive, passive agressive, histrionic, borderline) Also includes Developmental Delays (mental retardation, autism, language ans speech disorders, etc)

The Axis I diagnosis (or diagnoses) is the primary diagnosis for most clients. When the client has a depression, an anxiety, a bipolar, a substance abuse, an adjustment disorder, etc., this is where that information is put. When the client also has a personality disorder (or only a personality disorder), Axis II is where that diagnosis goes.

Axis II is only used to classify personality disorders and mental retardation.
Axis I is used to classify clinical disorders and other issues warranting clinical attention. It isn't just a dumping ground tho. If there is no Axis I diagnosis, it's usually V coded (since there is one providing for no diagnosis V71.09).
Axis III is used for medical conditions, Axis IV is for psychosocial and environmental problems (including relationship issues).

Axis I is used for mental disorders, like depression, schizophrenia, or anxiety disorders.
Axis II is used for personality disorders, like borderline personality, schizoid personality, etc. It is also used to list mental retardation.
