The softness of the bristle is determined by the diameter. A standard "soft" brush has 0.007" diameter bristles and these can slip under the edge of HEALTHY gums without doing any damage. Medium bristles are thick enough that they either will not fit under the gums or they will damage the gums if they do.

A soft tooth brush is recommended because you want to remove plaque on your teeth when you brush...this can be removed with your finger nail (that's how soft and easy it is to remove). A medium toothbrush is too hard for this purpose, and will only end up wearing your teeth (especially at the gumline, exposing sensitive roots)...I hope this helps....:)

A HARD OR MEDIUM TOOTH BRUSH WILL NEVER HURT THE ENAMEL, BUT THESE TYPES OF BRUSHES WILL WEAR AWAY THE GUMS AS WELL AS WHAT IS UNDERNEATH THE ENAMEL, THE DENTIN. THIS PART OF THE TOOTH IS VERY EASILY HURT WITH A MEDIUM OR HARD-BRISTLED BRUSH.
THE REASON PEOPLE BUY THESE TYPES OF BRUSHES IS BECAUSE THEY FOR SALE, BUT SO ARE CIGARETTES, ETC.
IF I, AS A DENTIST, HAD THE POWER TO GET RID OF ALL MEDIUM OR HARD BRUSHES IN THE WORLD I WOULD.
TOO OFTEN I'VE FIXED THE DAMAGE CAUSED BY THEM.

a harder toothbrush will wear away the gums, and cause them to recede, and so then the roots show up a little, and it makes your teeth more sensitive.
It can also damage the enamel of your teeth if you brush way too hard.
