I've read through the answers, and that one person who told you that it meant 'from the small hill' did not say that small hills are often considered gateways to the underworld, the home of the fairies.
I know a family called Twomey in Ballyvourney, County Cork. Lovely people.

Sounds as if it's origins may be in Tuomy or Tuamy. Possible connections with Tuam in Co. Galway.
