I don't understand any Chinese so I'm not sure what it's asking.

确认密码 is password confirmation?
保护邮箱 i have no idea what this means.
证件号码 no idea
提示问题 no idea
提示答案 no idea
验 证 码 and i'm guessing this is telling me to copy the image.

同意 or 不同意 is agree or disagree respectively?

Any help translating this form is greatly appreciated.

保护邮箱 i have no idea what this means. (Your email to confirm password if you forgot)
证件号码 no idea (a number you give so that with this number, hint question and answer to get back your forgotten password)
提示问题 no idea (hint question)
提示答案 no idea (answer for the hint question)
验 证 码 and i'm guessing this is telling me to copy the image. Yes.

同意 or 不同意 is agree or disagree respectively? (agree or not the user agreement and other conditions for use)

保护邮箱 is translated as "safety e-mail address" (huh?!) I think it means that when you lose your password, then they would send it to this e- mail address.
证件号码 is the card number (identification card, whatever card you have.)

验 证 码 is telling you to copy the letters from the box as you see it.
同意 and 不同意 is agree and disagree respectively
号为必填项，请您认真填写 means that the blanks that have the * or any indication nearby must be filled in, and please fill this in seriously.

You should get to the nearest Chinese Consulate e where you live and request them to translate it. Or you can find some on-line translator which for a reasonable fee will also translate it. But I would try first the Consulate.
