Continue to feed your puppy formula for now. Judging by your description, she does seem closer to the 3 or 4 week old. At three weeks they start to get up on their feet and wobble.
In a few days, start feeding her some "gruel". Take a puppy kibble, I use Eukanuba (buy small breed formula in a small bag) Also, purchase a couple cans of the wet food. Mix half wet food and get some of the dry food wet, let it soak for about 3 minutes, then mix that with the canned stuff. You can start out by putting a little in her mouth at a time. She should catch on pretty quick and start wolfing it down. This is also the time to introduce a water dish. She will probably step in it and spill it all over, but you can teach her to drink and she will need the water available all the time. During the next 2 weeks, start decreasing the formula and increase the puppy food. Then, try to have her on only puppy food and water in 4 weeks from now. When her teeth start to come in you can give her the puppy food dry.
As she grows, switch to a medium breed puppy food.
You are doing a great job by asking for advice and caring for one so small.
She will need a distemper series of shots. Usually they are started at 6 weeks old. I want you to wait until she is closer to 10 weeks old because she does not have any of the immunity provided by the mother's milk so she's going to be stronger and have more of an immune system if you hold off a couple extra weeks.

Keep the puppy formula going for another couple of weeks (if she is only 3 weeks at this time). Start pieces of bread soaked up in warm milk when you think she can handle solids. By the way, don't you have a Vet yet for her, to get her shots and stuff? The best person to ask would be the puppy's Vet.

Continue with the formula while you begin getting her to eat. I begin feeding all of my pups at 3 weeks, by mushing up puppy kibble with formula, and putting it to their mouth on my finger..soon she will begin eating it from a plate..
It is the right time to have her first de-worming..this is VERY important for any puppy to thrive..It should be done each 2-3 weeks, from the time they are 3 weeks old..
