There is no "male ballerina". It's a male danseur. Cody is correct, though I could of told you that off of the top of my head.

Depends on their rank in a ballet company. Terms such as Dancer, Principal Dancer, Soloist and Artist are generally what is used. The term ballerina is reserved for female dancers.

You can refer to them as ballerinas, even as strange as that may sound. But you can also use the term "premier danseur."
