To obtain a handi cap sticker (or placecard) for your vehicle, you first need to obtain a form from the DMV (or Secretary of State) and take the form to your physician (whatever one states you have a disability). Then you take the completed form back to the DMV (or Secretary of States office). They will process the form on the spot for you and issue you a handicap placecard. There are two types of handicap placecards. One is a temporary placecard and the other is a permanent placecard. In Michigan, a temporary handicap placecard is red, the permanent one is blue. If you are issued a permanent placecard, you can also ask for a handicap plate (in some States they will issue the plate right away, in others, they will send it to you) (either way, you still receive a placecard). You can use that placecard in ANY vehicle that you ride (or drive). Unfortunately, I was issued a permanent handicap placecard and license plate after being in a serious automobile accident.
I hope that this information helps you!

Usually the same office that does license plates will do the handicapped sticker. You will need a signed form from your doctor to get one, and they do expire. For conditions not expected to improve, you can get a permanent one in some states, check your local rules regarding that.

First you go to your doctor and have them give you a form that states your handicap and whether or not it is temporary, then you go to the license bureau, give them the form and they will issue you a handicap sticker.

After you get a letter or state form from your doctor, go to the DMV (or Secretary of State, depending on where you live) and apply for a handicap parking permit.

from your state government, usually from the motor-vehicle department, or whoever you get your license plates from, you will need to provide proof of eligibility
