If your HDTV has an HD tuner built into it, and you are in broadcast range of tv stations with HD you don't have to pay - but it may not be the programing you want. Frequently you will have to get an antenna for it (as long as it is UHF, it does not have to actually say HDTV on the box for it to work).
If your TV is advertised as being 'HD ready' you do need to provide an HD source such as from satellite or cable.

No, HDTV channels are free for the time being. On my tv remote I can get these channels by hitting the HDTV/TV button wait for a signal and I can get cineamax, encore, and local channels.

depends if you already have a cable service then change you tv remote to antenna and if you have a builtin HDTV monitor you might be able to pick it up free like i do on certain channels

It depends, if you have IO didgital cable, or any kind of cable, you don't have to pay extra for anything, they even give you the hd signal receiving equipment. If you have dishnetwork or directv, you have to pay extra for the service, and a lot for the equipment
