Well, you can't find out who his insurance carrier is, due to the privacy laws. Either you have a lawsuit and you have him served, and it comes out in court during the discovery period, or you don't. You can't file directly with his insurance company.
You CAN contact the New Jersey Medical Licensing Board, and do a search for a physician here: http://12.150.185.184/dca/simple_search.jsp
Your guy, btw, didn't come up. So my guess is he's either not licensed in NJ, or you spelled his name wrong.

the state medical board and check the local courts federal and local for suits. the dockets or case info is u sually public but not always.
