My grand father is above 70, his hands and legs have cracks, he is using homeopathic medicines and crack cream he is unable to get it cure can any one suggest the best medicine for reduction of cracks

tell him to use glycrine with rose water and apply some body lotion that is greasy.

Immerse feet in warm - hot water (whatever max tolerated) with a pinch of salt and some glycerine. When the skin softens, try to rub it off with a pumice stone.
When the skin is sufficiently cleared, apply mustard oil with a pinch of salt mixed. Then put on socks.
Alternaly you cna also apply liquid paraffin available in the chemists shop.
Repeat till feet are completely healed.
