Physical therapy has helped with raise my arms more, but the pain still exists. Do I need a cortisone shot? More physical therapy? Pain killers?

I would guess that you have Rheumatism in the joint or arthritis in the rotor cuff. The therapy may be making it worse, ask the therapist, then ask the doctor for a cortisone shot in the shoulder.
Didn't he take X-Rays of the shoulder or MRI's CAT's scans? he should have, he should have had an idea of whats going on in there to show you. You may have to have surgery to clean the cuff of the build up of the calcium.

You did not say how old you are but that does not matter. Shoulder pain can be coming at any age.

You are in front of the computer a lot. This is a guess but I know sitting in a chair that does not have arm rest causes this problem.

Here is what you do: Get up every 15 minutes to 30 minutes. Go for a cup of water or a (smoke..?!). When you type on your key board, rest your elbow on the armrest at all times. Your mouse should have a palm rest. A gel type palm rest is available in stores. It is cheap but effective. Buy one of those. Your keyboard must have palm rests at the bottom end. Otherwise, get one of those.

Alternatively, if you can give your computer work a couple of days of rest you will see things get back to normal slowly. Not doing computer work is not always possible for somepeople. Use your judgement.

Whatever you do, getting a comfortable chair with back lumbar support and armrests + keyboard and mouse supports are necessary. Adjust your monitor height to eye level. Looking down at the monitor puts pressure on your lower neck which radiates down to shoulders and arms.

Do all these and let me know how you find these suggestions.

Sounds like a pinched nerve. If your medical MD has cleared you for anything "serious"- why dont you try a Chiropractor. I used to get cervical (neck) and shoulder pain from lifting patients in bed. I took meds (minimal help). The first Chiro treatment-I was 70 percent better after only 1 adjustment. A good Chiropractor will do Xrays before adjusting you though. Try it....whats to loose.

in combination with the therapy you may need a light pain killer to ease the pain some times the physical therapy can inflame the muscles and joints to begin to hurt

Try getting a massage and working out your shoulders.

Instead you should do regular exercises and also see if massage or heating helps. If you can get an acupuncturist your problem will be solved.

Maybe you should change your mattress and your pillow. It can be causing you stress on your affected area.
