Suppose you got married on a fiance visa, legally, several years ago, but you never adjusted status to permanent resident. Could you still do it, or would it be too late? At what point would the fiance visa become worthless as a basis for adjusting status?

the fiance visa became worthless if you did not get married within the 90 days ........if the person got married to a US citizen ,which was the way to get the fiance visa anyway ......
since you did get married legally , there is not a problem ,
the person is not illegal in the US if the mariage did take place in the time allowed .........
please do not fear and write to them , if there is an adjustement to be made ?

Once the visa expires, it becomes worthless!

You are then "out of status" and subject to immigration arrest and deportation. Deportation can cause a bar to return.

However, if you go to Citizen and Immigration Services (CIS.gov) and start your permanent residence application then you will get a claims letter (if you are still married, if you are not you will probably have to leave).
