I can't answer this better than you doctor can, but if you want something over the counter that works well, I've found Maximum Strength Sudafed gellcaps to be the most effective over the counter decongestant,. I would also add that you should steer clear of over the counter nasal sprays as they are incredibly addictive.

Sinus,i.e. nasal allery cannot be treated permanently. Good medicines are available. Allegra pill. In India, this
is available in 120mg (to be taken twice in a day) & 180mg (once a day). In USA, it is in 60, 100 & 120mg.
Also good nasal spray brand AstraZeneca with regular
nasal spray is good. In India pills and spray are made w/o
steroids whereas in USA both are mixed with steroids. Mixed with steroids is expensive and results are better. However, the best solution is that one should consult with
ENT(ear, nose & throat) specialist. The above medicines are easily available over the counter. Now sinus is a common problem in all nooks and corners of the world due to mainly atmospheric polution, etc. One may visit Yahoo.com/health and search sinus & one can get detailed reply easily. However, heridity also plays its own role. The patient may consult an expert yoga instructor for nasal saline water practice and also a slim rope pulling thru nose with which one may not feel comfortable initially but may work well with long time use. However, for quick relief from sinus, one should consider the above two kinds of medicines, i.e. pills & spray, preferably, under ENT's instructions. Surrounding nasal area, one may suffer from different diseases, e.g. congestion, extra inner nasal bone, etc., for the treatment of which surgery may be necessary. If you are doing sneezes regularly with runny nose, Allgera & spray may be okay but if one is feeling nasal congestion, etc., one should consult an ENT. Finally, the best solution to sinus is that one should consult ENT before using any medicine.

I've had sinus problems since I moved from CA to IL- the thing that works best for me is Tylenol Allergy Sinus; even better than some prescription meds. Hope it helps. Best wishes.

First of all you should find out what foods/smell/dust irritates you and avoid them.
I had sever sinus problem for almost 20 years. Body weakness starts allergic reactions. Past three years I took homeo medicine from Dr. Batra's +heath clinic. It worked out from the day one. I am not sure it will work for you but you can try.

best medicine is figuring out what irritates it and avoiding that.
you could start eating fresh whole foods and that might help reduce allergic reactions.
make sure you've eliminated all the possible allergens in your house that you can. rip out carpets, clean out attics, throw out old papers or upholstery, run an air filter and in the winter a humidifier.
for infected sinuses, a warm saltwater rinse can help.
