My daughters birth certificate has our ages on it. So does mine and my mothers as well. I live in North Carolina, USA. All 3 are from here. I haven't ever seen one without the age of the parents on it.

I am in Oklahoma USA and yes, they list the age of the father (or atleast the birthdate anyway) they have for atleast 5 years since my son was born and 2 since my girls were born

In IN and OH, it gives the name and date of birth. At least it did with mine, with in the past 5 years.

In WI it still only says where the father was born no age yet that was as of 3yrs ago.
