In the US, if you have dental insurance through your employer, it may or may not be a covered expense. When you employer picked the dental plan, orthodontic coverage is an option they can purchase along with the package... or not.
Don't worry about cosmetic - if your plan covers ortho, it doesn't matter, contrary to what was written above. The typical orthodontic benefit is 50% up to a lifetime maximum (from $750 to $2000). It will cover Invisalign the exact same way as regular braces.

None. Braces are considered cosmetic and no insurance company will pay for them. Unless you have a medical condition (my younger brother had a cleft/palate, his braces were covered) there are no dental insurance programs that will cover braces.

insurance usually has a lifetime maximum on it for braces. I had Anthem and it paid the first $1500 for my daughters braces but will never pay anymore - the rest is my responsibility

I think it depends on the severity of your case.
If it's just cosmetic... I don't know, my HMO through Ohio SU's employee insurance paid for mine...
If it's really needed, like it interferes with your personal life in more than a "I just want to look pretty" way, most of them should pay completely.
