The clitoris since you can lose all sensation if anything goes even slightly wrong, ie. the piercer makes a wrong move or isn't experienced enough. The other that I don't understand at all is the eyelid.

Your question is too vague for me but I'll try and answer as best I can.
I love piercings, I have 10 of my own (only 2 in my lobes, use your imagination for the rest). For me the worst piercing to get would be one where the life expectancy of it is low such as a surface piercing. If I'm going to get a piercing I want my money's worth so something like a nape, temple, madison etc where they are more likely to reject is a waste of money and the time it will take to care for them.

Pierced with what? A sword through the heart is pretty painful. I had a nail go through my foot once. Youch! Other than that, I'd have to say any of your finger tips would be a painful place for a piercing.
