I cannot work do to a medical condition. How do I apply for unemployment and/or collect social security?

If you have a doctor's letter explaining that you can't work, then take it down to your Department of Human Services (or whatever state help in your state is called) and tell them that you want to file for disability. In the meantime, because from what I hear, that can take forever and a day, apply for unemployment and get that so you have money between now and when you'll start getting your SSI benefits.

FYI, the SS Administration will probably turn you down the first time. It's the way they weed people out since they know most people will become discouraged. When you win your appeal, the SSA will pay you from the date of the original application.

A medical condition is not usually a valid reason to apply for UIB. UIB is a temporary fund to assist people laid off or fired (yes, you can qualify for UIB if you're fired). You should check with your State's UIB Office.
