The rule of thumb concerning commas, is that you insert them where there is a break in the sentence, where things need to be separated, or where you would normally put in a pause.
For example, on the first one:
There is a pause after Shirley. Shirley is not part of the sentence, simply the person who is being spoken to. So there will be a comma afer Shirley. You also use commas to break up dates, so the date would be written Monday, December 12, 2004 and again, to partition the city and the state, so it would read Kansas City, KS.
A comma is also used after a title, and between the name and the title IF THE TITLE IS ABBREVITED AND FOLLOWS THE NAME, so the contact would be Terri Herndon, M.D.,
You would not, for example, use a comma between the title the name if the title came first. So Terri Herndon, M.D. is correct, but so is Dr Terri Herndon.

Using that as a guide, try the other three examples. In fact, if you can sort out the first one, you wont' have any trouble, ever, with where to put commas. That's a dandy example!

Whose to say those sources don't have typos either, but they do seem to be viable sorces.

1. False(I'm guessing on this one... I think specific ones are capitalized, but general ones aren't. "English" or "Spanish"(classes) are capitalized because they are names of language courses. Classes like "Math II" or "History 101" are capitalized because they are followed by numbers or roman numerals. Classes like "biology" or "science" aren't capitalized.)
