That sounds really unpleasant - is it worse during or around your periods? I would imagine you have already been for a pelvic ultrasound scan, as you know about the "fluid in the pouch of Douglas" - I type a lot of ultrasound reports at work, so it sounds familiar!

From personal experience and from typing those reports, I have noticed that this fluid can be left over from abdominal surgery, in which case it tends to clear up by itself. Or along with the pain you get, it could be a sign of a condition called Endometriosis, which I used to have - a type of bleeding during one's periods not only from the womb lining, but also from "wrong" places elsewhere in the pelvis.

There ARE helpful treatments available for this (including the right painkillers!) from your GP, or it might be worth asking for a referral to a Gynaecologist if you have not already seen one.

I do feel for you - although thankfully all that is behind me now at 52!

i had this after the birth of my first child. I was in absolute agony and its fluid that leaks into a 'well' between your back and front passage, then gets infected. I kept ignoring the pain and eventually ended up in hospital. This is so strange, cos I have never heard of ANYONE with the same complaint, and this was 12 years ago!!! Glad im not alone in this infection in such a strangely named area. Although I hope it clears up quick for you. Best of luck and by the way, it only took a couple of weeks and I was back to normal

it could be pelvic imflamatory diesese. Go see the doctor. The fluid could be something else and it might be your kidneys have you had a poo lately!
