All the answers are in the script and in the creative direction that the design team has gone in. Where is this production set? When? What does the costume design tell you about the makeup? What type of lighting will be used, and how will this affect your makeup design?

Your makeup design can't really be determined independent of the director's CONCEPT for the play. Is it being set in ancient Scotland, or is it being transposed in terms of time/place? Does the director want more realistic playing from the cast, or is he/she going for something more stylized? All of these things will figure into your makeup plot.

Just do basic stage makeup: base, eyeliner, blush, mascara, etc.
Macbeth isn't a glamor makeup show, really. If you chose to have the ghost of Banquo onstage instead of he traditional Macbeth-is-the-only-one-that-sees him, do gore makeup.
