The 'third world' has sometimes been described as the 'two thirds world' because it actually encompasses about 2/3 of the Earth's population - a useful reminder for us lucky ones in rich 'western' countries.

In theory, the "second world" used to be the countries where free enterprise was discouraged but the middle class ruled and everybody lived in harmony... no poverty, no oppression whatsoever. The so-called "second world" were the Communist countries, because Communism was meant to distribute wealth equally among the people so that there is no wealthy upper class oppressing the no-haves.
However, the world has seen that Communism has, in fact, failed. In practice, Communist governments tend to be very controlling to the point that the "equality" takes higher priority than the real welfare of the people but somehow 80% of the wealth that's supposed to be distributed among the people gets lost in the bureaucracy. Even Communist China has become increasingly open to a more free enterprise-oriented economy and other Communist countries have either collapsed (e.g. USSR), amalgamated (e.g. Vietnam, Germany), converted (e.g. Poland, Hungary, Croatia, and other Eastern European nations), or are in deep trouble (e.g. North Korea).
In practice, the "idealism" that "second world" promoters dream of is more clearly seen in first world countries where free enterprise prevails. In the so-called "wealthy" first world countries, most of the people are, in fact, middle-class and they have most their needs and some of their wants taken care of. Of course there are a minority that are poor and "oppressed", but they become like that because they let it happen to them. Also, there are a minority that are "filthy rich", but they become like that because it was their goal to become wealthy and they paid the price, drove the miles, to achieve that. But it's not so that the rich kick around the poor and treat them like animals, or the government tortures someone in jail for possessing something that's a little more expensive than what his neighbour has.
The "second world" doesn't work. That's why we don't talk about it any more.

The second world is nations that are fully industrialised and where the population is not dependent upon subsistence level farming, ergo famine is unlikely. It differs from the first world in that the majority live in poverty. However the definition of poverty is questionable and measures staple diets and ownership of goods or durables. Until recent times the second world included most of the Eastern Bloc despite the fact that the standard of living in East Germany was far superior to the equivalent social level in most South American countries.
