In order for the muscles to increase its size, increase the resistance and the intensity. You can try inclined push-ups, and increase the intensity by controlling the speed of each repetition.

Yes, this is true. Once you go over around 15 reps you start using slow twitch muscles, which are more for endurance. To stimulate fast twitch muscles you'll need to do weights that you can only lift for 8-15 reps at a time, maybe 3-4 sets.

The only thing you can do with pushups that I can think of is to have someone sit on you while doing them, or have a heavy backpack or something (probably not good for the lower back tho').

Pullups are actually a little better than pushups, if done correctly, because that's a lot of weight for the muscles trying to use them, namely lats, lower traps and biceps. This is why most people can't do nearly as many pullups as pushups, and why often skinnier people can do more (less weight to pull). But, your physique won't be as symmetrical with only one exercise, and gains will be limited since your body will adjust to that stimulus.

they will grow somewhat bigger, but you'll reach a plateau. endurance is great, and probably more useful for most people in daily life. if you want huge muscles, you've got to add weight to those exercises, or hit the gym.

Any strenuous exercise on muscles will make them bigger if you do enough of it
