Opetating cost's are hard to determine. every car acts differently. of course a small car with a 4cyl engine and a manual trans is your best bet.
as far as insurance: Saturn is generaly the cheapest on insurance.

My truck you see here (65 GMC) is only costing me $165 a year.
The truck my wife drives (97 Ford f150) cost $560 per year.
So would have to say the older the better.....and if it's paid off.
But the biggest factor in any insurance is your age, driving record and how long you had a license.
