These are governed by state regulations - you receive a copy of them when you enter into therapy. Some therapists choose to refuse to disclose information to parents, but that must be clearly stated within the forms that you sign when you request treatment.

Other two answers are correct. In many states, a separate authorization that you sign is required to allow the insurance company access to any psychiatric records as part of the underwriting process.
