Yes, I used to work in a nursing home and sometimes they'd bite down on the fork so hard when you were feeding them that when I finally could pry it loose, it would come out with their dentures on it.

What bothers me the most is when people suck their teeth and add a soundtrack while eating their meal. Hearing someone smack on their food makes me want to kick their teeth in. So in my opinion, those are definitely the two most annoying sounds.

Nah, but it drives DH crazy when I bite my straw and we're sharing a soda from McD's.

Sometimes. What really bothers me at the table is when people lick their fingers. That's a nasty table manner !
