Your employers should have a form that you sign on your return to work. It should state the days that you have been off. The reason and with most companies they ask if there was anything they could have done to prevent the illness.

I wasn't aware that there was an actual 'form' to fill in. Self certification is just telling your employer you're off sick without having an official sick note from the doctor. I think you can only do it for 5 days now without having to go get a sick note.

It is a Certificate signed by yourself that covers you for I think about 3 days sick... you can get these Certificates from Doctors surgery or chemist. If you are off sick for longer than this you will then require a Doctors Certificate. I hope you make a quick recovery!

self-certification means you sign a declaration saying you were ill without having to go to a doctor. Good for up to five days, but don't do it too often, or they'll ask questions.

From your or any doctors surgery. It's a form you fill in for up to seven days of sickness. After that you will be required to provide a doctor's note.

I'm not sure but i think it only covers you for three days.
i would get a doctors sick note just in case if i were you.
