Depends on what county you live in I think. In Missouri you can go to the post office, circuit clerk, or county clerk to get it...usually any government office but it seems to differ by county. In my county, the county clerk's office is the only place you can get one.
You can get the form online and fill it out or you can get it at the office you apply at. You must appear in person at one of those places with the form, money, a driver's license, original or certified copy of birth certificate if this is your first passport (just be aware that they will be sending this with the application), and two 2" x 2" pictures.

Go to any passport picture certified place such as a mailbox ect. type place, get your picture taken, then bring that along with your real birth certificate to the post office and pay the fee to have the passport issued.

1) Go to Walmart's Photo Center and get passport pictures taken.
2) Get a copy of your birth certificate
3) Go to post office--they will do the rest
