Everytime I bake cookies they are nice and puffy in the oven but when I take them out they fall, how do I keep them puffy?

Add more baking soda to it.

Add just a little bit of extra flour! If u r melting the butter for them dont! Let the butter get soft from being at room temp- never melt it or it will cause them to go flat! What I do is put one cookie in first and if its too flat I had a tbs more of flour until they come out soft and yummy! Make sure oven is preheated too!

try a little more flour, and stir the heck out of your cookie dough then let it rest in the fridge. the flour needs time to release it gleuten. gleuten is what knits the dough together. it helps to trap the gas bubbles inside. thus making for fluffly cookies, and bread.
but it sounds like you are using baking soda instead of baking powder. baking soda only releases gas once. baking powder releases gas twice.

Perhaps try an eigth or quarter teaspoon of baking powder.

If your dropping them off a spoon onto a cookie sheet, do not flatten the cookie batter.

The temperature of your oven may be off a little. Next time, try lowering the temperature a little (like 25 degrees), and underbaking them by about 2 minutes. Also, if you start with super cold dough, they will not flatten quite as much.

Could be you need to add more baking powder or soda to the dough; could be it's too old, must buy new; could be you're removing them from the oven too soon.

make sure that your baking soda is fresh, that will cause cookies to always go flat if its bad.
