I was thinking about the same thing the other day. It's funny how we hear phrases so much we know what they mean, but sometimes don't stop to think about it literally. I think it means to take something as if it will always be granted to you, as if you won't have to work hard for it in the future.

It is an old term that indicates that something has been granted to someones control and is their property, therefore they don't need to think about working to keep possession of them (as if Royalty has granted you something).
The modern term is usually used to indicate that someone has made the mistake of assuming that someone or something would always be there for them and as a result, didn't treat them with the proper respect. Often times it means that they were being arrogant in their assumption and/or they did not truly appreciate that which they "took for granted" and lost it as a result.

When something is "granted" to you it is something given to you and the implication is that you are fortunate for receiving it. So, taking something (or someone) for granted means you were expecting that thing (or someone's love and attention) because you think you're entitled to it and you expect to get it for free, without any effort on your part. Needless to say, that attitude doesn't win a lot of points with others.

It means expecting someone to do something without asking them or expect they feel a certain way without considering their thoughts/opinions. Love plays a big role in this situation. Just because someone marries you, doesn't mean he/she will feel the same way down the road without you doing your part in making the marriage work. You are taking that person's love for granted.

to have parents and loose them
to have a child and loose it
to have a high paying job and loose it
to have home and loose it
to be homeless and look back on all the things you lost
and then say I remember when but, that was then
now I appreciate and value all those things I lost only to say
'man I sure was stupid because I TOOK EVERYTHING FOR GRANTED'

Basically, it means to take or accept something without thanking for it or even realize that it's there.
Like life. (Almost) all of us take life for granted, aren't we?
