Mostly from chase planes but sometimes also with ground based cameras like they use for the space shuttle.
At the Royal Aircraft Establishment (Bedford) they used air and ground cameras synchronized by a broadcast timing code. The ground cameras were known as the Kine Theodolites and were located to the north and south of the main runway. You can still see one in the satellite image below, though the airfield is now used for storing cars and as a race track. You can see the shaddow of the dome that used to cover the camera the the tracking seats, two people steered each camera in azimuth and elevation.
The second image shows the other tower at the left end of the green shed, it was a bit hard to find.

They have another airplane fly in formation. Both pilots in command have to make arrangements with each other and cannot fly formation if they are carrying passengers for hire.

Most of the time they will chase the aircraft with another aircraft or helicopter so they can be at the same altitude of the aircraft to get go film of it.

Well, it's tricky because they can only do it in the baby off-season. See, the stork can't be outsourced to India (although he spends a lot of time there), but he can't make a good living just on babies. So he'll pick up a shot or two of a PanAm.
