If you find someone laying supine and the have vomit in their mouth. What is the first thing you do, open the airway or suction the vomit out? I am confused, I was always taught to open the airway (ABC), but if you open the airway first, can't you cause aspiration to happen.

Due to not knowing if the patient has suffered any spinal injury to clear the airway you need to log roll them over (roll entire body), to help clear some of the vomit out (if you don't have a suction device). However, you are correct except opening the airway is done by suctioning the vomit out. The vomit is acting as the obstruction in this case and by suctioning the vomit out opens your airway. you may want to suction out the vomit before opening the airway. if you don't do that and you open the air first then you will have a bigger problem on you hand instead of someone not breathing due the the fact that they have vomit in there airway. i may not be a doctor or a paramedic but i do know a lot about things like this. if your still no sure ask and maybe they might be able to further help you.

Logical thing is to remove what might be blocking the airway, which in this case...vomit can play a part into it.
You clear out any things in the mouth, such as the vomit, first then go on with the other steps.

Turn the Pt on thier side and clear out the vomit first , you have to remove the vomit in order to secure an open airway. if you can not turn them because of trauma then use suction.

If the vomit is not obstructing the airway, you need to secure the airway first, then you can sweep the vomit out of the mouth so that it does not aspirate.

Turn them on their side for their mouth to clear by gravity. If you open the airway and their mouth is full they will choke on their own vomit.

By clearing the air way means you stick your fingers in the victims mouth, and clean out any debris that is in there then give mouth to mouth with the head tilted at a 45 degree angle.
