A physician should allow you to keep you underwear on and if they want you to take them off I would be a bit suspicious. If there is a need to examine an area covered by your underwear it is very easy to pull it aside. I generally ask people to change into shorts and a sleeveless T shirt. Periodic skin surveys are an excellent preventive health measure as the incidence of skin cancer and melanoma are definitely on the rise.

Usually an examining gown is provided. Of course, if the involvement is very localized on hands and arms for example, complete disrobing would be unnecessary.

You will wear a paper gown and they undress the part they are looking at ......1 part at a time. If anything looks suspicious, they will let you know.
