If on a floor diagrapm it says the room is 12x16, and the room is mostly rectangle, but has a 2-3 foot long entry way..does that measurement take into consideration that entry way..or does the 12x16 measurement typically just the main rectangle shaped part?

Draw a line across where the entry way meets the main room, the calculate the sq footage of EACH square and then add them together.

Yes, it is still part of the room. If you are carpeting or tiling, of course you'd do these sections. To measure, treat the smaller section as a separate piece. Meaure the length and width of the main room for the area. Separately measure the entry. Add them together.

Go down cellar look at which way your floor joists run, their should one main beam running down the center of you house, this beam which is usually a large one, even steel, is the main beam, or load bearing beam.

I would draw it out on graph paper & then count the squares.
