I bought day planner inserts from Covey online, but you could make your own. Anyway, there are places to do menu planning, shopping lists, and also food journals. You can check off the kinds of exercise you do for the day as well.
Try to get an idea of what foods you should eat and what kinds you will actually eat. For instance, if you want to include more potassium in your diet then make a list of foods high in potassium (avacodos, bananas, blackstrap molasses....) and which of those WOULD you eat. Start to make an effort to include a food from your list everyday or several times a week.
Write it down or you are always playing the guessing game. "Did I take a calcium supplement on Moday ?!?"
Review how you are doing at the end of the week.
I have a list of items written on the refrigerator that I intend to eat daily. It helps to remind me when I am roaming in the kitchen.

Plenty of fish, chicken, salads, vegetables, unsalted nuts and fruit. Pasta is good in moderation too.
As for balancing your meals, it would be a bit lengthy to put in here but I'm sure you can find some nutrition guides on the web, so surf away.
Exercise is important too. Cardio exercise, and weights are beneficial, as is swimming and cycling.

Eat healthy by reducing sugars and carbs. Detox from time to time and excerise at least twice a week or walk daily for 30 mins.
