It's British English. It means "How are you?" and it is used only in the first time people meet eachother.
The answer to it is "How do you do?"

It is a formal greeting. Literally, it is "How are you", but it requires no answer. It is socially incorrect to answer it with anything other than "how do you do?" in return.
