You don't need a visa if you have a UK passport, they just give you a form to fill out when you're on the plane and you hand this in when you land.
If you have a criminal record you might need one though so if you've got a shady past best to check with the American Embassy which is in Grosvenor Square (just look for the millions of bollards and fences off Park Lane).

How could you loose a visa wich was stuck in your passport???
Anyway, if your passport is a biometric one (a new one with a chip in it, pictured on the first page) you shouldn't need a visa for the States, as a British citizen.
If you have an old passeport, my best clue is to apply for a biometric one.
If your visa was work or long stay, go to the American Embassy.

To replace a lost visa (how did you lose it if you still have the passport? They don't usually just fall out) you must apply in person at the US embassy that issued it. If that was London, you must make a new interview appointment, since non-interview cases must be able to present the previous visa. Here is how it's done:

You should be able to go to the uk embassy and state your case there and they should process your paperwork...in some countries a visa is a stamp on your passport though....

You do not need a visa to visit the USA. UK citizens can complete a Visa Waiver form on the trip into the USA.

You need to go to the American Embassy to get a replacement,i think that the Embassy is in the city (of London ) the Strand
