what do you do when a teacher assistant is left alone in the classroom with kids, and when a teacher withholds dessert from two kids? being all preschool teacher how do you handle these problems

Everyone, including all teachers, aides, and children need to be taught the importance of hand-washing. It needs to be a routine after using the restroom, and before snacks or lunch. If one of the parents happens to be a nurse, have him/her come in and give a lesson to everybody.
The director has the responsibility to address the issues you speak of. I don't know why the teacher withheld dessert, but children have a very acute sense of fairness; so, if it was an attempt to discipline the kids in question, it should have been handled differently.

This is usually handled by the director but if it is concerning the whole center sounds like training and staff meeting is needed.

A teacher that does not wash hands is in violation of health codes.

A teacher assistant that is left with the children as long as the ratio is kept that should be okay.

Withholding food should NEVER be an option this is in appropriate and the director should deal with this complaint.

Your director should be able to deal with these issues. If this person isn't any help then find out who your state monitor is (the person who issues your license and/or comes out to inspect) and email/call/write them with your concerns.
