Well, there are lots of reasons, but the major ones come from within. It could just be that you're scared of getting hurt, or that you just aren't ready to make that leap. If you're coming out of a relationship that went sour, some of those residual emotions could also make you scared. I think the important thing is to try to understand why you feel the way you do, and don't be pressured into anything.

Sometimes you can be nervous because you already like them so much and you are afraid that you might be so boring that you will never get another shot. Of course the opposite happens too... you already know someone is into you and you are afraid that while you like them OK that after one date they are going to just want all of your time and attention.

If they're acting like you're already a couple when you've barely met.

If they show ANY temper at all, like raising their voice or breaking things.

If they talk about their private parts or mention sex before you've even gone out.
