I'm just wondering, does appendicitis start with pain? For those of you who have had it, was there any kind of discomfort first, or was it just all of a sudden you had pain? I'm just wondering how it all starts.

You may not get a pain, some don't some just get ill, toss their cookies and run a fever.

Sometimes a doctor cannot tell an appendix attack without a test, a urine sample and the while cell count. This is unless you have an unusual violent attack and your abdomen is hard enough to stand on and stiff and you can't straighten out your legs. This doesn't happen that often really.
So, if you think your having an attack you might want to get to an ER and let them decide, they may want you to wait between checks to see if your white count through stages changes, this is another way to tell. They have to make sure.

Appendicitis can come on slowly or all of a sudden. Pain below your belly button on the right side is a good indicator. If the pain does not go away, I would go to your doctor. He will probably draw labs. An elavated white blood cell count and a positive CAT scan will verify. Becareful, a lot of CT departments use RECTAL contrast in their appendicitis protocols.

Appendicitis is caused by a blockage or infection of the appendix, or a ruptured appendix from a blow to the abdomen. The pain often happens quickly, becoming excruciating within hours. Treatment must be immediate, because serious complications and/or death can result.
