There is no one recognized formal ceremony. However there are some common practices.
The only requirement is that the flag be treated with respect and disposed of by burning.
Most ceremonies involve cutting the Union (the blue field and stars) from the stripes. In this way it is no longer a flag and can be burned without disrespect.
In formal ceremonies usually some narative is read or recited. I've seen each star cut away and placed in the fire separatelhy while the names of the states were called off. I've seen the Union placed whole into the fire to symbolize national unity.
Do it with dignity and respect and you've met the requirements of 4 USC.

all of these are helpful tips. The fact is that there has been no established guidelines for a flag retirement ceremony at all. So this has been largely left up to the imaginations of the people. Some prefer highly symbolic ceremonies (cutting off the blue field, and the stripes individually and then burn the pieces), others prefer folding the flag and laying it atop a fire. Whatever you do, make sure it is done courteously and with the utmost respect. Good luck on your project, from a current Eagle Scout to a future Eagle Scout.

fold the flag properly.
make a funeral pyre (log cabin-type fire does well)
light the fire.
place the flag on the fire.
make sure that all pieces of the cloth are burned thoroughly and turned to ash. scatter the ash when you are done.
it is usually appropriate to have a ceremony with the troop and discuss any number of topics. some appropriate ones are:
1. how to be a good citizen
2. remembering veterans
3. discussing our freedoms and how we choose to use them
etc....
i wish you the best in your service project. :)

the flag should be lowered reverently, and disposed of by burning in a dignified manner.
