Although this may not be causing the mold/mildew in your home, I feel compelled to warn you that if you notice the windows in your home producing a lot of condensation ( then running onto the walls and mildewing) this winter when the heat is on,please get your furnace vent checked to be sure it is not plugged with bird nests. We went through a winter not long ago where the windows dripped moisture and didn't know why until months later when we found our furnace vent pipe packed with bird nests. We were fortunate that the carbon monoxide was not lethal but it would have been by the next winter,I'm sure.

Windows, generally moisture from sweating and being open and getting wet from the outdoor moisture sources, namely rain.
Ceilings could be a leak(s) or possibly overinsulation when the original construction was performed (can't breathe). A building can be overinsulated and condensation forms on interior surfaces.

Not enough ventilation. Do you have roof vents? Fans in the bathroom and kitchen to move moisture out?

Or if you live in a very moist environment, get a household dehumidifier.
