My character is being slowly poisoned by his father. His father is putting something in his medication. I was imagining a strong cleaner. What do you think? What would the bodily reactions be? I was just planning on him experiencing gut and intestinal pain. Is that enough or should I have him feel something else?

rat poison can take a long time to kill if you are planning to have your character suffer for a while... especially if the dosing is not quite right. it is also readily available.

Ex-lax in a high dose, I believe, will give off lots of cramping and in need of something to catch the explosions. They will be uncontrolled urges and should be very painful.
