Never:

My wife and I have three kids. Two of the three were conceived using birth control (the pill and yes she was taking it properly). The third was conceived using a condem.

Specifially what you are asking for is using the natural family planning method. With that you must monitor in detail temparature assaciated with various days of the month to determin when she is ovulating.

Even then there is no garantee.

My adivice: If you cant handle being a parent then sex is not worth it. Yes sex is great and at the most you may enjoy it for a couple of hours. However raising children sucks, costs a lot of money, and ruins your social life. Trust me its not worth it.

just wait.

Even if she's been on the pill, she needs to be sure she's taking it at the same time of day, and not skipping or forgetting a pill, or its effectiveness decreases. But a second method of birth control (ie, condoms) is a good idea too. Nothing is 100%.

The thing about human females is that we are fertile all the time. If she is on the pill then he chances of getting pregnant are significantly lower. Like a 1% chance. If you wear a condom, then not only would you take care of that pesky 1% but you would also be mostly protected from STDs.
