Look little lady the post office can give you the forms required to get a USA passport. If you have the funds there are several sites that will help you get your passport. Fees range from $150 to $200. Here is a good web site www.globalvisa.com They also have a toll free number to call and they will be more than glad to help you obtain an USA passport. ( This is what I would do, so much easier and simplier).

Many (if not most/all) US Post Offices offer passport services, so if you need to renew your passport or just obtain a new one I'm sure the Post office representatives at the counter will be able to help you out. Good luck!

Yes, many post offices offer passport services for a fee. You can also look online for one that offers this service in your locale.
That's where I got mine - from the local post office (rush service is additional).

You only take passport pictures and fill in forms at a post office and they lodge it off for you. You do have to pay a fee.
I recently lodged mine with them.

It's way easier/convenient if you just visit http://travel.state.gov/passport/passport_1738.html and get your application, fill it out, the look for a place closest to you, were you can go apply. I just got mine about 20 days ago!

Yes, they will give you an application form, which you have to complete and send off with two photo's, birth certificate and get someone to countersign both the form and one of the photo's to confirm it really is you. (England) Oh yes, forgot about the money, £65!
