Do you mean that you want to put your Name and number in a header above your normal Text on the Page? If so use the Header/footer under the View heading .. you can put it up as a tool bar on your word tool bars.

Go into the View tab and select Headers and Footers. When it pops up, you can put whatever you specify in there. For the page number, make sure that you use the "Insert Page Number button that shows up from the header/footer taskbar.

You're talking about a header. In MS Office 2003, you go to insert.. header. In MS Office 2007, you go to the "Insert" tabs on top, then Page Number, Top of Page, and Page Number 3.

Click on View, then Header & Footer. Then, type in your name in the upper footer (flush right). There'll be a little button with the page # on it on the header/footer toolbar that you click on to add the date. Otherwise, you can click on Insert, then Page Number, and check the box next to Upper Right. Hope that helps!

It has been awhile since I have done this but I believe you go to header under view at the top. Make it custom.

Go to "view" "headers and footers." There you can pick left, middle or right and type anything in you want.
Hope that helps.
