By asking "Smallpox" do you mean the vaccination or the actual disease? You seemed to have asked that like someone asking "Have you ever had heartburn" Smallpox is a pretty serious virus and doesnt usually affect civilized societys as a matter of fact i dont know of anywhere in the world where there are lots of documented cases anymore. But yes i have had the vaccine i got the newer version of the vaccine about 10 years ago. it gives you a little scar at the site of the innoculation. The old vaccine use to give you a huge scar the size of dime or so. The scar is from the disease actually entering your body you actually get one of the "pocks" thats how you build immunity to it.

Yeah Sassy is right only people that get it now are Military and travelers to third world places.

Smallpox was eradicated a looonnnnggg time ago. So long ago, that children aren't even given immunizations for it anymore. However, those of us who are older were immunized before we went to public school at age 5. I still have the scars on my arm. But...never had smallpox! Did have chicken pox and that was bad enough. I believe the small pox immunization may be required for anyone traveling abroad; I'm not sure. Godloveya.
