I know they mean medications, but I'm allergic to shellfish, should I still tell them that?

YES. They don't mean just medications. They need to know everything. There are medications that include food products that people may be allergic to or that are generally known to be a problem for people who are allergic to certain foods.

Yes you should. Medications are made from many different things and just in case there is something in a medication that mimics shell fish, they should know about it. You never know what is in the compound that makes up the medication they may have to prescribe for you. You should always tell them anything you have an allergy to. They can sort out what is important and what isn't. If you decide that it isn't important enough to mention and you are wrong, it could mean severe complications in the long run.

It might be a little silly, but it never hurts to be completely honest with everything you are allergic to. Many doctors do specifically ask (on intake forms) to list EVERYTHING you're allergic to, and they ask for meds that you are allergic to on a separate line. This results in me personally telling them that I am allergic to shorthair cat dander, mold spores, and ragweed pollen. [note: although penicillin is from mold, I am not allergic to it or any drug in its family.]

Another interesting idea (I credit my dad for this), is to mention medication intolerances to the nurse when s/he asks you what meds you are allergic to. Most of us have some meds, or even entire classes of meds, that we do not get along with, due to side effects. For example, I do not get along with "dopamine antagonists"... these meds include some anti-vomiting drugs and pretty much all antipsychotics. Regardless of why the doctor might give me such a drug, I have a very nasty anxiety+depression psychological reaction to them. That is just an idea, though, and you probably want to tell the nurse that it isn't an allergic reaction per se, and is actually an otherwise intolerance.

Hope this helps...

why don't you ask if food allergies are something to worry about too. and if they are then go ahead and say that you are allergic to shellfish.

They usually mean medication, but if it is a hospital stay, you may want to tell them of your food allergies as well
