I have the same problem as you when it comes to learning French, and the trick is not to get too impatient with it. Make absolutely sure that you understand each bit of grammar, or you can't advance. Also, take your dictionary and aim to learn 5 new words each day. Write down lists of words to remember them, and try practising with friends. Still in my opinion, the only way to fluently speak a language is to be forced to speak it, so I would recommend a visit to an English speaking country for a while.

Wordreference.com provides a dictionary from English to several other languages (you didn't say which your first language was) but I advise that you check this out, because it provides a list of common phrases related to the word you type in.

Good luck!

Do NOT watch MTV, I would suggest going to a nearby library and checking out a computer program to learn the english language. And don't worry too much, you seem to have a better handle on it than many who are born into it. Watching english language films will help, especially if you have someone watching with you that is fluent in english to ask questions of. :)

Continue your reading, listening, writing and speaking english practice. I'm not english and follow this rule. Study, also, a little bit of basic grammar! Good english language knowledge will come with practice.

expand your circle of english friends. Its only by speaking the language often that you will learn.

listen to radio 4 (you can get it streamed, online)
It is a discussion/arts/comedy channel and there is less slang than you will find in other media.

your English is already better than any of my (2) foreign languages.

Talk to people as much as you can - watch TV programmes and read. Try to keep all communication to English.
That will help.
