The problem for you is that JFK is isolated from the city. New York Traffic being what it is with lots of jams, construction and stop lights, you should figure on spending about about an hour to get where you want to get so that you are safe. You will be wanting to return in the evening during rush hour, so add a half hour to your traveling time. You will want to be back there by 6 for a domestic flight, 5 for an international.

So assuming your flight is International, you could have from 11:00am to 3:30pm actually enjoying Manhattan.

Your best bet is to take taxi cabs to and from Manhattan. (Manhattan is the part of Manhattan that everyone goes to.) There was a law passed a few years ago saying that a taxi fare can't be higher than about $33 if it is from or to the airports as long as it is within the city limits.

As for what to do - that depends on what you like.
What do you like to do? Shopping? Fifth avenue near central park and Canal street near china town. Museums? Pick either The met or moma. Being a tourist? Take one of the bus tours. Do you want to hang out in a hip area? Soho, Greenwich Village and The East Village are your answers. Are you an architecture buff? Hire a guide to take them through the major land marks. Do you want a broadway show? Head to times square. If food is important, then get the Zagat guide to restaurants. Also, consider buying the current issue of Timeout New York. It has what is going on every week.

If you really want to leave the airport and head into Manhattan, I would pick ONE thing that you really want to do and arrange it ahead of time.

For example - to get the half price tickets at the TKTS booth, you can sometimes wait on a really long line.

There can also be long lines at the Empire State Building, etc. Try and find something that has timed admission so you know exactly when you will get in and out.

Also, if your plane leaves at 7, you need to be there by at least 6, which puts you travelling back to the airport right in the middle of rush hour! Rush hour starts around 4:30 and can be quite heavy on some parts of the trip to the airport.

Definitely leave yourself enough time to get back there, go through security again, etc. There is no place at the airport to leave your baggage, so you will have to keep it with you.

Tab a taxicab to Manhattan.....and yes, you do have time for an afternoon show....have the cab take you to "Tkts" on Broadway(that's around 47th/48th and Broadway....you can get inexpensive tickets to an afternoon show or and off broadway show too......plenty of places to eat too. If you have time yes, the Empire State is not far away from there.
If you think that might be too hectic......have the cab take you to Central Park and take in the scenery......the Metropolitan Museum is right there too. (I would start at 59th and 5th Central Park East)......you can see the Plaza Hotel is right there, many, many stores.......FAO Schwartz....etc. Madison and and 5th Avenues are just plain fun to walk down and window shop and people watch.

Get a ride into NYC and visit the Empire State Building or Broadway (catch a musical or something)

Take a cab to Aqueduct Racetrack. Play the ponies and have a nice meal.
